Search for a way to improve your memory and you will be sold something within about thirty seconds. Brain supplements are a multibillion-dollar market built on a genuine desire, a plausible-sounding story, and remarkably little proof. The pills are usually legal, usually safe, and usually useless.

That is a strong claim, so here is the evidence behind it, along with the short list of things that do have some support. Nothing here is medical advice, and anything you swallow with the intention of changing how your brain works is worth discussing with a doctor first, especially alongside other medication.

What are nootropics, exactly?

The word was coined in the 1970s for substances that improve learning and memory while being essentially non-toxic. In practice it has become a marketing umbrella covering four very different categories, and lumping them together is the source of most confusion.

There are everyday stimulants such as caffeine and nicotine, which have real, measurable, short-term effects on alertness. There are prescription drugs such as modafinil and ADHD medications, which work but are medicines with real risks and are not the subject here. There are herbal and dietary supplements, the bulk of the consumer market, where the evidence is mostly weak. And there are vitamins and minerals, which reliably help only if you are deficient in them.

That last point drives more of this market than anything else. Correcting a genuine deficiency of B12 or iron can produce a dramatic improvement in thinking, and supplement marketing borrows the credibility of that effect to imply the same thing will happen to people whose levels are already normal. It does not.

Why the market outruns the evidence

Supplements in most countries are regulated as food rather than as medicine, which means no one has to prove a brain supplement works before selling it. Nobody checks in advance, and the burden falls on regulators to act after the fact.

The result is a predictable style of claim: supports brain health, promotes mental clarity, helps maintain memory function. Those phrases are chosen precisely because they are unfalsifiable and legally safer than saying improves memory, which would require evidence. The US Federal Trade Commission publishes guidance on health product claims setting out what advertisers are actually supposed to be able to substantiate, and the gap between that standard and the average label is instructive.

Enforcement happens, and it tells you how much of this is real. Supplement makers have faced similar action over memory claims. A product being on sale means only that it is on sale.

What actually has some support

Being fair to the field, a few things do have evidence, with heavy caveats:

  • Caffeine has the strongest and least disputed short-term effect on alertness and vigilance, though its benefits and limits are narrower than most people assume, as covered in our guide to whether caffeine helps you focus.
  • Correcting a deficiency works, reliably, for B12, iron, vitamin D, and folate. The prerequisite is being deficient, which a blood test establishes and guesswork does not.
  • Omega-3 fatty acids have decent evidence for long-term brain health and weak evidence for improving cognition in healthy, well-fed adults. Eating fish twice a week is better supported than the capsules.
  • Creatine shows small effects on some memory and reasoning tasks, most clearly in vegetarians and sleep-deprived people, which fits the pattern of correcting a shortfall rather than adding capacity.
  • L-theanine with caffeine has modest support for reducing the jittery edge of caffeine while preserving the alertness, which is a real if unspectacular benefit.

Notice the shape of that list. What works either corrects a deficit or is a stimulant. Nothing on it adds capability to a healthy, rested brain, and that is the honest summary of the entire field.

What does not hold up

Ginkgo biloba is the most instructive case, because it was tested properly. A large, long, well-designed trial following thousands of older adults for years found that it did not prevent cognitive decline or dementia. The National Center for Complementary and Integrative Health summarizes the research on ginkgo without enthusiasm, and it remains a bestseller regardless.

Most multi-ingredient brain formulas fall down for a more basic reason: they combine eight to twenty ingredients at doses often below anything ever tested, in proportions nobody has studied, on the theory that a longer label reads as more science. Racetams, popular in online nootropic communities, have little quality evidence in healthy adults and are unapproved for that use in most countries. Coconut oil, various mushroom extracts, and most memory blends sold in pharmacies have promising early laboratory findings and no convincing human trials, which is the normal state of an ingredient that will never be shown to work.

A useful habit when reading any claim: check whether the study behind it used humans or cells in a dish, whether the participants were healthy or already impaired, and how many there were. A great deal of supplement marketing rests on findings from rodents, from very small samples, or from people with a genuine deficiency, none of which transfers to a healthy adult buying a bottle.

Is any of it risky?

Sometimes, and the risk is easy to underestimate precisely because supplements feel harmless. The three real concerns are interactions, contamination, and dose. Ginkgo and high-dose fish oil affect bleeding and matter if you take blood thinners or face surgery. St John's wort interferes with a long list of medications including contraceptives and antidepressants. Because oversight is light, independent testing regularly finds products that contain less than the label claims, or extra ingredients it does not mention.

So the sensible rules are unremarkable: tell your doctor and pharmacist about everything you take, be suspicious of proprietary blends that hide doses, prefer products with third-party testing, and treat any promise of dramatic improvement as a reason to walk away. Anyone whose memory or thinking has genuinely changed should be investigated rather than supplemented, since the treatable causes, thyroid problems, deficiencies, sleep disorders, medication side effects, all require a diagnosis to fix.

What would you spend money on instead?

If the goal is a sharper brain, the interventions with strong evidence are unfortunately free, which is exactly why they are unpopular. Sleep is the single largest lever anyone has, and no supplement comes close to the effect of moving from six hours to eight. Regular exercise has the best evidence of any intervention for long-term brain health, with effects visible in brain structure. None of that sells well, because it cannot be packaged. The rational version of a supplement budget is to spend it on a blood test to rule out the deficiencies that actually matter, and put the rest toward whatever makes sleep and exercise easier to keep up. That is a duller answer than a bottle promising clarity, and it is the one supported by the evidence.
